The following photos are of spalling on concrete. This is often caused by a weak surface layer due to overwatering the surface while finishing. The weaker layers degrade faster when exposed to freeze thaw cycles. The fix recommend here was to abrade the surface and apply a repair mortar to resurface the areas and finish with a broom. The repair mortar suggested was H2.
